Trefn Cofrestriad / Registration Proceedures
Os hoffwch gofrestru hefo Meddygfa Rhydbach yna dewch a cerdyn meddygol hefo chi y tro nesaf yr ydych yn dychwelyd i'r feddygfa neu gofynnwch wrth y gweinyddes an ffurflen GMS1. Pan yr ydych yn cofrestru hefo ni, mi fyddan yn awgrymu eich bod yn gwneud apwyntiad i weld y nyrs am siec meddygol.

If you wish to register with Rhydbach Surgery, then please either bring your medical card with you or ask the receptionist for a medical form GMS1 to enable you to register with the practice. When registering with us, new patients are invited to make an appointment with a Practice Nurse for a health check.

System Apwyntiad / Appointment System
Fe all apwyntiadau gael eu bwcio o flaen llaw ganiatad y bod apwyntiad ar gael. Os ni allwch fwcio apwyntiad o flaen llaw hefo'ch Doctor arferol yna ffoniwch y feddygfa o 8:00yb ar y diwrnod. Mae gan pob claf gyfrifoldeb i ddod i'r apwyntiad unwaith y mae o wedi cael ei wneud.

Appointments can be booked in advance providing there is an appointment available. If you are unable to book an appointment in advance with the GP of your choice, you can ring the surgery from 8:00am on the day. Patients have a responsibility to attend an appointment once booked.

Cyfrinachedd / Confidentiality
Mae gan oll weithwyr y feddygfa fynediad i wybodaeth personol ond y maent yn cael eu rheoli gan reolau strict o gyrinacholdeb. Fydd gwybodaeth ddim ond yn cael eu drosglwyddo i 3ydd parti hefo caniatad ysgrifenedig y claf o flaen llaw.

All practice staff have access to personal records but are bound by strict rules of confidentiality. Information is only to be passed on to third parties with the patients prior written consent.

Rhyddid Gwybodaeth / Freedom of Information
Mae manylion ar reolau rhyddid gwybodaeth y feddygfa ar gael wrth ofyn. Mae cynllun cyhoeddiad yn ganllaw i'r gwybodaeth y mae'r feddygfa yn bwriadu eu wneud ar gael yn rheolaidd.

Details of the practice Freedom of Information publication scheme are available on request. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make available.

Mynediad i'r Anabl / Access for the Disabled

Mae ganddom ni fynediad hawdd i bobl anabl a chyfleusterau toilet da.

We have easy access for the disabled and good toilet facilities.

Ymddygiad Ffyrnig ac Ymosodol / Violent and Aggressive Patients

Yr ydym yn rholi polisi di-oddefiant tuag at ymddygiad ffyrnig ac ymosodol. Ni fydd ymddygiad ffyrnig nac ymosodol yn cael eu ddiodda yn y feddygfa am bod diogelwch ein cleifion yn holl bwysig. Bydd cleifion trosyddol yn cael eu gofyn i adael y feddygfa ac eu tynnu oddi ar ein rhestr.

We operate a zero-tolerance policy towards violent and aggressive patients. Violent or aggressive behaviour will not be tolerated in the practice, as the safety of other patients is paramount. Offending patients will be asked to leave the premises and will be taken off the list of their registered doctor.
